Biometric Screenings and Wellness Incentive Program May 17-July 19

This year, there are some important updates to the Wellness Incentive Program.

The program updates help provide participating team members with resources that support their wellness journey. The Wellness Incentive Program now includes a biometric screening with applicable Care Management and an online wellness video.

To participate, employees and their spouses who are enrolled in a health plan through Kettering Health on either the high deductible health plan (HDHP) or the engaged PPO plan must do two things during the period of May 17-July 19:

1. Complete a biometric screening
2. Watch the wellness video on HealthStream (employee only)

By completing these requirements, engaged PPO plan members can earn a lower premium in 2022, and high-deductible health plan (HDHP) members can earn an employer contribution to their health savings account (HSA) or flexible spending account (FSA) in 2022, if not eligible for an HSA.

Your action is necessary to receive the benefits of the Wellness Incentive Program for your 2022 medical health plan prices and premiums. Follow the instructions below for details on completing both components of the program.

How to complete a biometric screening

The biometric screening can be completed at Kroger Health, with your primary care provider, or at one of our On-Demand Care locations. To complete one of these options, please follow the directions below:

The Little Clinic (available May 17-July 19)

1. Print the Kroger Wellness Customer Voucher and 2021 Physician Screening Form.
2. Visit www.thelittleclinic.com to schedule an appointment
   • Choose category “Preventative Services” and “Biometric Screening” for the reason when scheduling online
   • Appointments are strongly encouraged due to COVID-19. Walk-ins may be subjected to long wait times.
3. Arrive at your appointment with a completed Kroger Wellness Customer Voucher, 2021 Physician Screening Form with section 1 completed, and a valid photo ID.
4. Submit your completed (sections 1 and 2) 2021 Physician Screening Form via email to Ketteringhealthplan@ketteringhealth.org or fax to (937) 522-9190.

Kroger Pharmacy (available June 8-July 19)

1. Print the Kroger Wellness Customer Voucher and 2021 Physician Screening Form.
2. Visit www.krogerscreenings.com or call 1-877-444-9689 to schedule an appointment (first time patients will need to register).
3. Arrive at your appointment with a completed Kroger Wellness Customer Voucher, 2021 Physician Screening Form with section 1 completed, and a valid photo ID or fax to (937) 522-9190.
4. Submit your completed (sections 1 and 2) 2021 Physician Screening Form via email to Ketteringhealthplan@ketteringhealth.org

Primary Care Provider

1. Have your physician complete section 2 of the Physician Screening Form.
2. Send completed (both sections 1 and 2) Physician Screening Form via email to KetteringHealthPlan@ketteringhealth.org or fax to (937) 522-9190.
   • Please note: Only examination and blood work completed from September 13, 2020 to July 19, 2021 will be accepted on the Physician Screening Form.

If an employee had all the bloodwork completed that is listed on the physician form between September 13, 2020 to July 19, 2021, they can print their MyChart results, sign the physician form, and submit that to complete the activity.

On-Demand Care Location

1. Print the Kettering Health On-Demand Care Biometric Screening Voucher
2. Visit access.ketteringhealth.org/biometric-screenings/ to schedule your appointment
3. Arrive at your appointment with the complete voucher form and a valid photo ID

How to Watch the Wellness Video in HealthStream

1. Log into HealthStream via myHR portal
2. Search the catalog for “2021 Wellness Incentive Campaign Education”
