Patients require an order and appointment.
The order must include the following:
- Patient’s Name
- Date of Birth
- Diagnosis
- Order number (98416), if sending a patient to the COVID drive-thru at Southview or Huber
- And it must be signed by the physician.

EMPLOYEE HEALTH
A requisition from Employee Health is still required. Employees do not require an appointment, although it is highly encouraged.

- Please click on the COVID-19 black box to go to the COVID section.
- Then, click on the Testing banner at the top of the page.
- Then, select the site.
MyChart
- Log into MyChart.
- Go to the “Visits” icon at the top of the page.
- If you are using the MyChart App, alternately click “Appointments.”
- Select “Schedule an Appointment.”
- Select “Covid Drive-thru Testing.”
- Answer the questions.
- Select the location.
- Select a date and time.
- Click “Schedule.”
