The Emergency Department at Fort Hamilton is providing patients a new level of service after they go home.
As ED Navigator, Diane Perkins, RN, BSN, works with certain discharged ED patients to connect them with local physician follow-up care and appointments. She calls approximately 30-40 patients per day to discuss how their care was in the ED, discuss follow up and preventative screening, and answers any questions they may have.
“The best part of my job is to know I am satisfying the needs of our patients,” says Diane. “Navigating health care can be a challenge and it is great to know I am making a difference.”
