In July 2018, Kettering Medical Center began a journey to redesign the care delivered to patients, creating a perioperative surgical home (PSH) program.

The perioperative surgical home
• Is a patient-centered, physician-led model of care • Optimizes patients’ health before surgery, actively involving patients in their care
• Focuses on standardization through enhanced recovery pathways
• Utilizes coordinated, interdisciplinary teams to manage patients through all phases of care
• Tracks patient progress through the surgical experience with a safe transition back to the primary care provider

The perioperative surgical home team at Kettering has been meeting monthly to plan and implement the perioperative surgical home. The team includes social workers, hospitalists, surgeons, and staff from perioperative as well as inpatient nursing units, Anesthesia, Process Excellence, Nutrition Services, Pharmacy, and Information Systems. The team has developed protocols and patient education materials
and constantly measures the impact through a data dashboard.

The initial patient population included in the perioperative surgical home was colorectal, with the recent addition of gynoncology. The outcomes have been very positive. Length of stay, complications, readmissions, narcotic usage (see graph) have all decreased. Patient satisfaction has improved as well. Through the PSH, clinicians are bringing the community the right care at the right time while reducing costs.

Members of the PSH team have had the opportunity to present at the AORN national meeting in April as well as the American Society of Anesthesiologists PSH Learning Collaborative meeting.
