To order your shirt, visit the Kettering Health Network Rewards Store at ketteringhealth.org/rewards and log in to your account. Then follow the process below:
- Click the box that says “Click Here to Order Your Fortune T-shirt.”
- Select your size in the drop-down box.
- Click “Add to Cart.”
- Proceed to checkout. The shirt should cost 0 points.
Once you place your order, your t-shirt will be sent directly to the shipping address you provide. All orders must be placed by September 6.
First time logging in to the Rewards Store? Enter “KHN” plus your badge number for your login ID (ex: KHN12345). Your password is the first four digits of your birth date and the last four digits of your social security number (ex: If your birthday is April 9 and the last four digits of your Social Security Number are 1234, your password is 04091234).
