If you (and your spouse, if applicable) are enrolling in the 2018 Engaged PPO Plan, you can qualify for a reduced wellness premium for 2018 by completing a free biometric screening at a Kettering Health Network location, followed by any applicable care management. This is part of the Wellness Incentive program. On-site biometric screenings will be offered at various Kettering Health Network locations between October 30 and December 8. Details on specific dates, times and locations are available when you login to the biometric appointment scheduling site.

If you’ve participated in a biometric screening previously and have already created an account in the Interactive Health scheduler, you can login again using the same email address and your DOB. If you are a new participant, you will need to create an account.

What does it cost?
Kettering Health Network is providing the biometric screenings at no cost to you or your spouse if enrolling in the Engaged plan.
If I’m enrolling in the High Deductible health plan can I still participate?
If you’re enrolling in the 2018 High Deductible Plan, you completed your screening earlier this year in the Spring to earn your 2018 incentive. Kettering Health Network will offer biometric screenings for the High Deductible plan enrollees again in early 2018 to earn the 2019 incentive. More information will be provided soon!
Do I need to bring anything with me to the screening?
If you are a 2017 Engaged PPO Plan member, please bring your Health Plan Card with you.

Are the on-site screenings my only option?
If you have seen or will see your physician and have the same tests completed as the on-site biometric screenings, you can utilize a physician form. To receive a physician form please contact Employee Connect or visit your campus HR.
