At this time, we are not releasing medical records in person at our Prestige Place location in Miamisburg. Medical Records can be obtained in the following ways:

- Downloading the form found on our website at https://www.ketteringhealth.org/patientrelations/medicalrecords/ and faxing it to (937) 522-8444
- Via MyChart, for patients that already have access
