The COVID-19 Workforce Survey is a brief, five-minute survey focused on employee safety, resilience, support, and communication. The feedback you share through this survey will help us understand what’s working well and where we can make meaningful adjustments or improvements to support our teams as this situation continues to evolve.

Is my response confidential?
This survey is confidential, and it will not be possible to identify your individual response. Those who take the survey will be asked to provide high-level demographic information that will help us understand and respond to feedback. You will not need to enter a badge number or access code to take the survey.
How long is the survey open?
The survey will remain open through Friday, May 1.
