As coronavirus (COVID-19) continues to evolve, we have made it our goal to keep you informed as we have updates to share.
Patients that are being discharged from our facilities may have a COVID-19 rule-out test that has been performed and before the test results are received. In addition to the verbally informing the patient/family of the pending test, a SmartPhrase should be added to the Discharge Instructions in the After Visit Summary (AVS). The SmartPhrase is noted below.

PENDING LAB RESULT
You have pending lab results for potential COVID-19. Please note, we are required to report any testing for COVID-19 to Ohio Department of Health (ODH). We will also notify ODH of your discharge from the hospital so they can continue to follow your results and provide guidance as applicable. Your PCP Primary Care Provider (PCP) will receive your medical records from your hospital stay along with lab results once they are made available. Please note, testing can take 3-5 days for results. Your PCP will provide further instructions regarding your treatment plan once test results are received. If you have questions or your symptoms worsen, please feel free to contact your PCP (Ohio Department of Health is available to answer questions as well). During this timeframe, we ask you self-isolate and utilize this link “What to do if you are sick with coronavirus disease 2019” educational sheet we have provided and reviewed with you regarding self-isolation.
