This Friday, October 15, is deadline for employees to submit medical or religious exemption requests related to required flu and Covid-19 vaccinations.
Below are reminders about this process and how employees can access the appropriate forms.
- Employees who need to apply for an exemption from both the flu and Covid vaccine requirements will need to submit separate request for each vaccine.
- Religious and medical exemption forms are available on the Employee Health intranet page or in myHR. This includes temporary exemption requests related to antibody testing, trying to become pregnant, pregnant, or breastfeeding.
- Once forms are complete, they should be submitted by email to EHPrograms@Ketteringhealth.org. Please title the email to include the type of the exemption followed by your last name, first name, and badge number. This will help our teams get your request to the right committee for review.
With the deadline approaching, we are receiving a high volume of exemption requests, and team members may receive a response to their exemption request after October 15. All requests submitted by the October 15 deadline will be considered; each review committee is working diligently to review and respond to requests in a timely manner.
